I start my covers @ 5 x 5" - 300 DPI (Dots Per Inch) RGB/CMYK

Some people do the 4.75 x 4.75" but I add extra space for safety.

When the cover is done, and is template ready, I basically save the files as Photoshop EPS Format in CMYK (For Illustrator) and go from there.

Also depends on the printer. I had a customer who said he needed the file in RGB. Some require different bleeds as well. Start at 5"X5" 300 dpi. And no, it's not good to have already started your file at 72 dpi and resize. It will get distorted.
