Not yet. I filled it out last year but the claim form changed sites so i filled it out again a few weeks ago.
Im either getting 2 checks for the same price, 1 check, or i get denied because i filled it twice lol.

yup, It seems that you don't even have to have a receipt, and for now you can get reimbursed for up to five jars if you bought them between Jan 1, 2008, and Feb 3, 2012.

^ You haven't needed a receipt to get money for all the ones in this thread. They are filled out with no proof of purchase and you get a check in X amount of months. Now if you DID have an actual receipt on most of them you could get more money than with no PoP though.
