correct

but I do my artwork in RGB it gives me more flexibility, when I'm done with my design I select all, copy ALL layers, create new file, paste then change the mode to CMYK and don't let it flatten your image. CS2 and CS3 are very good at keeping the colors looking the same.

Different printers have their own templates, although they may not be too much different in dimension. I use DiscMakers' template for my designing and will transfer them to the template provided for the printer (if given one).

You can find DiscMakers' templates here. Or you can just make your own by creating a file 5"x5" and putting a guided at .25" on the top and left, and 4.75" on the right and bottom (that's how I use to do it).
