you can use pretty much anything. i use illustrator/photoshop to do my shirts. check with your silkscreener or printer though to be sure. they may have a preference, whether it be a hard copy or an .eps (photoshop/illustrator format).

as far as templates... silkscreens usually are 12x14(inches). so your printout, or file, should fit that.
