The photo is pretty sick. Out of those the best one is the first one, but I'm not sure about the text. The others looks horrible, trash them immediately. The background on the first one looks the best, but it would be more suitable if say there were some hospital beds in the back, making it convincing. Your dimensions look way off, you can't design a cover like that. Design the cover @ 4.75 x 4.75" @ 300 DPI CMYK, and leave a .25" bleed making it 5 x 5".

If the title is "The Prescription," then Im seeing like a pharmacy or somethin. Something that gotta do with medicine and doctors.

my vote goes for the 1st one except for the"The prescription" Font and style
also the background needs to be filled as samos commented or
maybe as if hes in a morgue where they hava them metal things where they store the dead bodies after autopsys
just an ideas
but the actual photo of him is clean though
