Then look for ones that you don't need proof of purchase for, fill out the claim, forget about it, and then 6-12 months later receive a check and buy beer.

the claim form asked for the last 4 digits of SS#, seems fishy?

Some of them have asked for that in the past. You can't do anything with just the last four of someone's SS# anyways it's just used to verify the person is who they say they are just like when you call Comcast or your bank.
