Resizing the pics

I see a few folks doing this and I always wondered how its done without losing quality? Because having like a 400 x 600 pixel image of someone (just a example coming off the top of my head) and resizing it to fit a cd cover which is basicly 5 x 5 inches?

the quality looks the same when u size it down but if you try and resize it bigger then it will get blurry

I try but sometimes some photos I just cant do shit with them, I can work on them for hours and get them looking decent, and I hate decent.
